As a means to participate in value-based contracting and shape local health care delivery, the Care Centered Collaborative is assisting physicians and physician organizations interested in forming clinically integrated networks (CIN).

At the core these CINs will be the quadruple aim – improved quality outcomes, lower cost, patient satisfaction and provider satisfaction. The CINs will be physician-led and develop / participate in value-based contracts within the commercial insurance market.

Better workflow, better coordination of care, and less dependence on revenue from filling hospital beds are key factors that may help physician-led ACOs thrive.

The Collaborative’s role is to provide opportunity for physician aggregation without consolidation. This will be accomplished by providing administrative and financial support within these CINs for care management programs and a data platform - the most expensive resources needed to successfully operate as a CIN.

Currently Collaborative efforts are underway to form CINs, including a statewide structure linking CINs under one umbrella CIN.
